Key Responsibilities:

Care Advocate:
Coordinate and facilitate the transition of patients from healthcare facilities to home health services, ensuring a smooth and stress-free process.

Relationship Developer:
Build and maintain strong relationships with hospital staff, including discharge planners, case managers, and other healthcare professionals.

Patient Supporter:
Evaluate patients' needs and ensure all necessary arrangements are made for a seamless transition to home health services.

Knowledgeable Educator:


Inform patients and their families about home health services and the transition process, empowering them with essential knowledge.


Communication Liaison:
Act as a liaison between healthcare facilities, patients, and home health teams to ensure continuity of care.

Sales Advocate:
Conduct outreach and presentations to promote our home health services to healthcare facilities and providers.

Collaborative Team Member:
Work closely with the home health care team to create and implement personalized care plans for patients.

Detail-Oriented Professional:

Qualifications:

Current LPN or RN license in the state of Oklahoma.

Experience in a healthcare setting, preferably in a hospital or home health environment.

Strong interpersonal and communication skills.

Sales-oriented mindset with the ability to build and maintain relationships with healthcare professionals.

Ability to work independently and collaboratively.

Excellent organizational and time management skills.

Valid driver's license and reliable transportation.

Familiarity with home healthcare services and referral sources.

Ability to navigate electronic medical records (EMR).
