Position Overview:

The Discharge Coordinator plays a vital role in ensuring a seamless discharge process for patients, particularly those in critical care settings. This position operates under minimal supervision and requires a proactive approach to patient care.

Key Responsibilities:

  • Designs and implements a thorough discharge program that promotes effective transitions for patients in the CICU, collaborating closely with healthcare professionals including physicians, nursing staff, social workers, and case managers.
  • Manages the inpatient discharge process, providing essential education to families and ensuring their informational needs are adequately addressed.
  • Evaluates the discharge program's effectiveness and proposes necessary modifications to enhance patient outcomes.
  • Ensures compliance with established policies and procedures while providing necessary information and support to patients and families.
  • Collaborates with medical staff and families to formulate a comprehensive care plan that addresses the patient's needs upon discharge.
  • Trains nursing personnel and physicians on the documentation processes related to discharge, core measures, and overall discharge procedures.
  • Maintains accurate records of the discharge process to ensure continuity of care.
  • Identifies and coordinates post-discharge healthcare resources for patients and their families as needed.

General Duties:

  • Performs additional responsibilities as assigned by management.

Qualifications:

Education: A Bachelor's Degree in Nursing is required.

Experience: A minimum of two years of experience as a Registered Nurse, preferably in Pediatric Critical Care settings.

Licensure/Certifications: A current RN License issued by the Oklahoma State Board of Nursing or a valid multistate compact RN License (eNLC) is required. Basic Life Support certification from the American Heart Association is mandatory upon hire, and Pediatric Advanced Life Support (PALS) certification must be obtained within six months of employment.
