Reports to: Program Manager
Schedule: Monday - Friday, 8:00am - 4:30pm

SUMMARY: The Housing Stability Case Manager plays a vital role in promoting Clients' housing security, self-sufficiency, and integration into the community through evidence-based methodologies. This position involves collaborating with individuals experiencing homelessness to identify housing challenges and implement strategies for maintaining housing. The Case Manager also provides training in independent living skills to facilitate the transition to permanent housing.
DUTIES and RESPONSIBILITIES:
- Manage a caseload of Clients with significant needs
- Foster relationships that prioritize independence and health enhancement
- Work in conjunction with mental health and healthcare providers to improve treatment coordination
- Execute interventions that prioritize Client autonomy, harm reduction, and rehabilitation
- Assist with the screening, intake, and enrollment processes for Clients
- Conduct assessments and develop treatment plans
- Document and maintain progress notes and data
- Coordinate services and appointments for Clients
- Aid Clients in accessing benefits and strengthening community ties
QUALIFICATIONS:
- Bachelor's degree preferred or relevant experience
- Valid Driver's license and reliable vehicle required
- Familiarity with case management preferred
- Comfortable engaging with Clients facing various challenges

WORK ENVIRONMENT:
- Combination of fieldwork and office setting
- Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs
