Job Overview

The Mental Health Association - Western MA is seeking a dedicated Case Manager to join our team. The ideal candidate will engage in outreach, assessment, and support for participants in our Low-Threshold Housing Program, utilizing a trauma-informed and person-centered methodology. This role is crucial in assisting individuals recovering from substance use and facing homelessness to secure and maintain stable housing while addressing their comprehensive needs.

Key Responsibilities:

Intake and Assessment

  • Facilitate the necessary documentation for program entry, including homeless verification and substance use assessments. Prepare initial Adult Comprehensive Assessments and treatment plans.

Service Development

  • Collaborate with a multidisciplinary team to create Individualized Action Plans that leverage participant strengths to achieve their goals. Document progress and adapt strategies as needed.
  • Design and implement crisis management and behavioral plans in partnership with participants and the treatment team.

Housing Support

  • Assist participants in securing safe housing and foster relationships with landlords.
  • Provide tenancy support, including negotiation for repairs and compliance with tenant responsibilities.
  • Help participants organize moves and provide physical assistance as necessary.

Employment and Benefits Assistance

  • Support participants in obtaining and maintaining employment, training, or educational opportunities.
  • Educate participants on accessing benefits and resources such as Social Security and SNAP.

Community Engagement

  • Encourage participant involvement in community resources and activities, promoting self-determination and recovery.
  • Empower participants to utilize community resources for improved life skills and social engagement.

Health and Wellness Support

  • Provide emotional support and guidance on coping strategies and harm reduction.
  • Assist in coordinating medical and psychiatric appointments, ensuring participants receive necessary care.

Team Collaboration and Professional Development

  • Actively participate in team meetings and training sessions to enhance skills and knowledge.
  • Maintain professionalism and effective communication to support participant and program goals.

Safety and Compliance

  • Identify and address safety hazards, ensuring a secure environment for all participants.
  • Report any incidents or concerns in accordance with established protocols.

Qualifications:

  • Valid driver's license and reliable vehicle.
  • Bachelor's degree in human services or a related field.
  • Experience working with individuals facing mental health or substance use challenges is preferred.
