Key Responsibilities:
Participant Engagement and Advocacy
1. Interact with participants in a respectful and motivating manner to bolster self-esteem and encourage self-direction.
2. Guide and assist participants in various daily activities, including personal hygiene, meal preparation, and maintaining their living spaces, while promoting their involvement.
3. Tailor support levels to meet the diverse needs of participants, ensuring their comfort and safety.
4. Facilitate access to community resources, including educational institutions and social organizations, to promote independence.
5. Empower participants through self-advocacy training and act as a liaison with healthcare providers as necessary.
6. Provide guidance on financial management, including budgeting and banking, while ensuring proper documentation of participant funds.
7. Encourage community integration by supporting participants in joining local organizations and engaging in recreational activities.
8. Ensure participants have transportation for essential appointments and social engagements, maintaining a reliable vehicle for this purpose.
9. Research and propose community events to enhance the activities calendar.
Health and Wellbeing Support
10. Assist in evaluating each participant's abilities and needs to inform their Individual Service Plan (ISP) and implement strategies accordingly.
11. Respond to emergencies in line with established protocols, keeping relevant personnel informed.
12. Report any incidents of participant abuse or neglect in accordance with agency policies.
13. Support participants in managing medical appointments and medication, documenting health-related progress as needed.
14. Aid in grocery shopping and meal planning as directed by supervisory staff.
Documentation and Reporting
15. Complete all necessary documentation within specified timelines, including daily logs and incident reports.
16. Communicate any issues or concerns promptly to supervisory personnel and maintain accurate records.
17. Utilize online reporting systems and attend training to enhance technical skills.
18. Prepare monthly progress notes for participants, reviewing and documenting their development.
Safety and Compliance
19. Maintain a safe working environment by identifying hazards and reporting incidents as per safety policies.
20. Assist participants in emergency evacuations and conduct regular safety drills.
Collaboration and Professional Development
21. Participate in team meetings and training sessions to enhance job performance and professional growth.
22. Exhibit professionalism and effective communication to support program goals and participant needs.
23. Engage in the training of new staff and assist in scheduling adjustments as necessary.
Qualifications:
Essential Skills and Experience
- Valid driver's license and reliable vehicle.
- Ability to work independently and collaboratively as part of a team.
- Strong communication and organizational skills.
- Experience with individuals with developmental disabilities is preferred.
Education:
- High school diploma or equivalent; a degree in human services is advantageous.
- Minimum of six months of relevant experience required.
