Key Responsibilities- Conduct initial assessments and develop individualized service plans to address the unique needs of clients.
- Provide information and referral services to clients, connecting them with community resources and services.
- Collaborate with healthcare providers and community organizations to ensure seamless care coordination.
- Assist clients with completing applications and paperwork related to medical coverage, housing, and disability benefits.
- Provide education and support to clients and their families on HIV/AIDS transmission and risk reduction.
- Advocate for clients and provide support to help them maintain independent living.
To be successful in this role, you will need:
- A CHW Certification, Associate's or Bachelor's Degree in a human services field.
- At least one year of experience working with HIV/AIDS or substance-use populations.
- Bilingual (Spanish/English) ability preferred.
- Excellent verbal and written communication skills, interpersonal skills, crisis intervention skills, and organizational skills.
