Job Overview

The Mental Health Association - Western MA is seeking a dedicated Case Manager to join our team. This role involves providing outreach, assessment, and engagement to participants of the BSAS Low-Threshold Housing Program through a trauma-informed and person-centered methodology. The Case Manager will assist individuals in substance use recovery and those facing homelessness to secure stable housing, maintain their residences, and address their various needs.

Key Responsibilities:

Intake Coordination

  • Facilitate the necessary documentation for program entry, including homeless verification, substance use assessments, eligibility screenings, and authorizations. Prepare initial Adult Comprehensive Assessments and treatment plans.

Service Planning

  • Collaborate with a multidisciplinary team to create Individualized Action Plans that leverage participants' strengths in achieving their goals. Document progress and support the implementation of strategies outlined in the IAP.
  • Design and implement program support strategies, crisis management plans, and behavioral interventions in partnership with participants and the treatment team.

Housing Stability

  • Assist participants in accessing safe housing and foster relationships with landlords.
  • Provide tenancy support, helping participants negotiate necessary repairs and maintenance, and ensure compliance with tenant responsibilities.
  • Support participants in organizing moves, including utilities and address changes, and assist with packing and transporting belongings as needed.

Income and Benefit Maximization

  • Guide participants in securing and maintaining employment, as well as accessing educational opportunities that lead to job placement.
  • Educate participants on available benefits and resources, including Social Security, EAEDC, SNAP/Food Stamps, and Fuel Assistance.

Community Engagement

  • Encourage participant involvement in community activities by facilitating access to libraries, recovery groups, and local events.
  • Empower participants to utilize community resources to enhance self-determination and support their rehabilitation and recovery.

Health and Wellbeing Support

  • Provide emotional support and guidance in developing coping skills, while counseling on harm-reduction strategies.
  • Assist participants in coordinating and attending medical, dental, and psychiatric appointments, and support them in establishing nursing services.
  • Ensure crisis accessibility during shifts by monitoring communications and responding promptly to urgent situations.

Team Collaboration and Professional Development

  • Actively participate in team meetings and training sessions to enhance job knowledge and skills.
  • Demonstrate professionalism and effective communication to meet the goals of participants and the program.
  • Provide transportation to participants for essential appointments and engagements, ensuring vehicle readiness and compliance with safety protocols.

Safety and Compliance

  • Promote a safe work environment by identifying hazards and reporting incidents as per established protocols.
  • Document and report any incidents of participant abuse or neglect in accordance with agency guidelines.

Qualifications:

  • Valid driver's license and reliable vehicle with appropriate insurance.
  • Ability to work independently and collaboratively within a team setting.
  • Strong communication, organizational, and interpersonal skills.
  • Experience with individuals facing mental health and substance use challenges is preferred.

Education:

  • Bachelor's degree in human services or a related field is required.
  • Minimum of six months of relevant experience preferred.
