As part of our Next Step/Housing First initiative, you will play a crucial role in ensuring that participants have access to safe and sustainable housing. You will empower individuals by providing the necessary support and encouragement to engage with their community effectively. The Mental Health Association is recognized as a top nonprofit organization and an excellent workplace, offering a supportive environment for all employees.
This position involves a blend of office work and field engagement, making it ideal for a dedicated individual passionate about community service and committed to assisting participants in securing permanent supportive housing. The working hours are from 9 AM to 5 PM, Monday through Friday.
Key Responsibilities:
Service Planning:
- Conduct comprehensive assessments to better understand and engage participants regarding their unique strengths and experiences.
- Utilize a client-centered approach to assist participants in developing and implementing an Individualized Action Plan (IAP).
- Provide ongoing follow-up with participants to support them in achieving their goals outlined in the IAP.
Housing Stability:
- Guide and motivate participants to maintain their housing units in accordance with lease agreements and housing quality standards.
- Assist participants in negotiating with landlords for necessary repairs and maintenance.
- Coordinate annual inspections and address any immediate needs for unit inspections.
- Build and sustain positive relationships with landlords.
Financial Empowerment:
- Support participants in securing a reliable source of income through employment or entitlements.
- Educate participants on accessing benefits and resources, including Social Security, EAEDC, SNAP/Food Stamps, and fuel assistance.
Health and Wellbeing:
- Assist participants in scheduling primary care and mental health appointments to promote overall well-being.
- Encourage adherence to medical and counseling appointments by connecting participants with wrap-around services.
- Provide transportation to ensure participants can meet their medical and social service needs.
- Facilitate referrals to community programs and services to foster meaningful connections.
Development and Additional Functions:
- Attend required meetings, supervision sessions, and training programs.
- Complete necessary documentation, including assessments and progress notes.
- Communicate and document incidents promptly and accurately.
- Perform other related duties as assigned.
Qualifications:
- A valid driver's license, sufficient automobile insurance, and a reliable vehicle for work-related tasks.
- Ability to work independently, manage time effectively, prioritize tasks, and collaborate as part of a team.
- Strong oral and written communication skills, along with good organizational abilities.
- Flexibility to adapt to the evolving needs of program participants.
- Excellent interpersonal skills, demonstrating patience, compassion, and empathy.
- Minimum age requirement of eighteen years.
Education and Experience:
- High school diploma or equivalent (GED) is required; a bachelor's degree is preferred.
- Experience working with individuals with disabilities who have faced homelessness or mental health/substance use challenges is essential.
- Previous experience in assisting individuals with housing acquisition is preferred.
- Bilingual candidates are encouraged to apply.
