Job Summary

We are seeking a skilled and compassionate Patient Care Coordinator to join our team at Lifespan Corporation. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to our patients and their families.

Key Responsibilities
  • Collaborate with Registered Nurses to organize work and set priorities.
  • Perform personal care skills, basic nursing assistant skills, and rehabilitation skills as outlined by the Rhode Island Department of Health.
  • Maintain a safe environment by reporting unsafe conditions and situations to Registered Nurses or supervisors.
  • Recognize and report signs and symptoms to Registered Nurses.
  • Accurately document patient data in the Electronic Health Record (EHR) and maintain quality assurance and infection control.
Requirements
  • Current Rhode Island license as a Nursing Assistant.
  • High school graduate or equivalent preferred.
  • Ability to perform basic computer skills.
  • Certified in American Heart Association Healthcare Provider Basic Cardiac Life Support (BLS).
Working Environment

This role requires prolonged sitting, standing, and walking, as well as repeated bending, squatting, and stooping. May be required to lift or turn patients and equipment.
