Position Overview

The Behavioral Health (BH) Case Worker: Family Partner is tasked with delivering structured, individualized family support and training services to parents and caregivers, focusing on the needs of the designated youth client. In collaboration with an identified clinical hub, such as an Intensive Care Coordinator (ICC), Outpatient or In-Home Therapist, the Family Partner plays a crucial role in the Family Resource Center (FRC) alongside a licensed clinician. The Family Partner is instrumental in executing the client's care plan, which encompasses service eligibility assessments, service plan formulation, resource development, and providing education, coaching, advocacy, and support.

Key Responsibilities


• Enhance the caregiver's ability to effectively support the youth, aiming to improve the youth's community functioning or facilitate their reintegration into the community. This includes education on navigating child-serving systems (such as DCF, education, mental health, and juvenile justice), fostering empowerment through peer support, and identifying community resources (like after-school programs and food assistance).


• Deliver high-quality care to clients, adhering to best practices and agency standards while maintaining ethical guidelines and privacy standards.
• Exhibit professionalism and compassion in all interactions with clients. The case worker should embody reliability, integrity, respect, and a commitment to ongoing learning and collaboration. Professional attire and effective communication skills are essential.


• Maintain or exceed established caseload expectations, which may require flexible working hours.
• Accurately document all services in the required data entry system, ensuring compliance with established protocols and timelines.


• Address any concerns related to client, staff, or programmatic risk immediately with appropriate supervisory personnel.


• Embed core values in practice, including client-centered care, recognizing diverse perspectives, and promoting equity and inclusion.
• Attend and engage in various meetings and training sessions to foster professional growth and development.

Additional Duties


• Represent the agency at community events or committees when necessary.
• Collaborate effectively with colleagues and supervisors, contributing positively to the agency's workforce.
• Assist supervisory staff with data and information related to quality and compliance as needed.

Qualifications


• Demonstrated proficiency in youth and family casework, with knowledge of mental health, educational, and child welfare systems preferred.
• Valid driver's license and reliable transportation are required for fulfilling job responsibilities.


• Ability to establish therapeutic rapport with clients and build collaborative relationships with service providers.


• Proficiency in care planning, coordination, advocacy, and discharge planning is essential.
• Strong oral and written communication skills, along with organizational abilities and a collaborative spirit.

Physical Requirements


• Capable of performing sedentary tasks, including desk work and driving, as well as engaging in therapeutic activities with clients.
• Must be able to intervene safely when necessary, using approved techniques to ensure client safety.
