The Behavioral Health (BH) Case Worker: Family Partner plays a crucial role in delivering tailored, one-on-one family support and training services designed to meet the unique needs of youth clients. In collaboration with a designated clinical hub, such as an Intensive Care Coordinator (ICC), Outpatient, or In-Home Therapist, the Family Partner is dedicated to facilitating the implementation of the client's care plan. This includes determining service eligibility, developing service plans, coordinating services, and creating resources while providing education, support, coaching, training, and advocacy.
Key Responsibilities
• Support caregivers by enhancing their ability to effectively parent the youth, thereby improving the youth's community functioning or aiding their reintegration into the community. This involves offering education, navigating child-serving systems (such as DCF, education, mental health, and juvenile justice), fostering empowerment, and connecting families to peer support and community resources.
• Deliver high-quality care to clients, adhering to best practices, agency standards, and ethical guidelines while maintaining privacy and HIPAA compliance.
• Exhibit professionalism, ethical practice, and compassion towards clients. The case worker should demonstrate reliability, integrity, respect, and a commitment to ongoing learning and collaboration. Professional attire and effective communication, both verbal and written, are essential.
• Maintain or exceed established caseload expectations on a weekly, monthly, and annual basis, which may require flexible working hours.
• Accurately document all services in the required data entry system, following established timelines and protocols. Respond to quality management and utilization review alerts as necessary.
• Report any concerns related to client, staff, facility, or programmatic risk to a supervisor or licensed senior staff member immediately.
• Embed core values in practice, including promoting client-centered care, recognizing diverse perspectives, and maintaining a strengths-based approach while being mindful of safety and risk issues.
• Represent the agency at community events or committees when appropriate.
• Collaborate effectively with colleagues, supervisors, and other personnel within the organization, contributing positively to the work environment.
• Assist supervisors with data and information related to quality, compliance, and productivity as needed.
• Conduct case presentations or consultations within the scope of expertise.
• Perform other duties as assigned.
• Demonstrated proficiency in individual/youth and family case work or wrap-around care, with knowledge of mental health, educational, developmental care, child welfare, and juvenile justice systems preferred.
• Valid driver's license, acceptable driving record, and reliable transportation are required to fulfill job responsibilities. Vehicles must be properly registered and insured according to state requirements.
• Ability to establish therapeutic rapport with clients and foster collaborative relationships with service providers.
• Experience as a caregiver for a youth with special needs is beneficial for teaching and supporting parent/caregiver consumers.
• Proficiency in care planning, coordination, service delivery, advocacy, and after-care planning is essential.
• Strong oral and written communication skills, organizational abilities, and a collaborative spirit are necessary.
• Ability to perform sedentary tasks, including desk work, typing, and driving to meetings and trainings, as well as transporting clients when necessary.
• Capability to engage in therapeutic activities with children and families.
• Must be able to fulfill the duties listed above, with or without reasonable accommodation.
• May be required to intervene to ensure client safety using approved techniques as necessary.
