Position Overview

The Family Support Specialist will play a crucial role in delivering tailored, one-on-one support and training services to parents and caregivers, focusing on the unique needs of the youth client. In collaboration with designated clinical teams, the Family Support Specialist will assist in executing the care plan for the client. This encompasses eligibility assessments, development of service plans, coordination of services, and resource identification, alongside providing education, support, coaching, and advocacy.

Key Responsibilities


• Enhance the capabilities of parents and caregivers to effectively support the youth, thereby improving the youth's community functioning or facilitating their reintegration into the community. The specialist will guide caregivers in navigating various child-serving systems, empowering them through connections to peer support and community resources.


• Deliver high-quality care that aligns with best practices and agency standards. Uphold ethical guidelines while maintaining confidentiality and adhering to privacy regulations.
• Exhibit professionalism and compassion in all interactions with clients. This includes demonstrating reliability, integrity, and respect, as well as maintaining a neat and professional appearance. Effective communication, both verbal and written, is essential and should reflect the agency's policies.


• Meet or exceed established caseload expectations on a regular basis, which may require flexibility in working hours.
• Accurately document all services provided in the designated data management system, adhering to established timelines and protocols. Respond to quality assurance and review processes as necessary.
• Promptly report any risk-related concerns or ethical issues to appropriate supervisory personnel.
• Embed core values in practice, including client-centered care, recognition of diverse perspectives, and a strengths-based approach. Remain aware of safety and risk issues throughout service delivery.

Additional Duties


• Represent the organization at community functions or committees when appropriate.
• Collaborate effectively with colleagues and supervisors, contributing positively to the organizational culture.
• Assist supervisory staff with data and information related to quality and compliance as needed.
• Participate in case presentations or consultations within the scope of expertise.
• Perform other related duties as assigned.

Qualifications


• Proven or developing expertise in youth and family case management, with knowledge of mental health, educational, and child welfare systems preferred.
• Valid driver's license and reliable transportation are required to fulfill job responsibilities. Compliance with state driving regulations is mandatory.
• Ability to establish therapeutic relationships with clients and collaborate effectively with service providers.


• Experience as a caregiver for a youth with special needs is advantageous for coaching and supporting other caregivers.
• Competency in care planning, coordination, advocacy, and discharge planning is essential.
• Strong oral and written communication skills, along with organizational abilities, are necessary for effective teamwork.

Physical Requirements


• Ability to perform desk work, including computer tasks, and transport clients as needed.
• Capability to engage in therapeutic activities with children and families.
• Must be able to fulfill job duties with or without reasonable accommodations.
• Training in safety and de-escalation techniques is required to ensure client safety.
