Patient Intake Specialist

Obran Cooperative is in search of a dedicated Patient Intake Specialist. In this position, you will oversee the intake procedures for new patients, ensuring that all essential information is collected and accurately recorded.
As a key member of our healthcare team, you will contribute significantly to the seamless operation of our home health services.
Your key responsibilities will encompass:
- Evaluating and processing referrals from healthcare providers, patients, and their families.
- Collecting all pertinent information to commence the intake process, including introductory calls with patients.
- Establishing and nurturing strong connections with case managers and referral sources.
- Collaborating with healthcare professionals to arrange initial assessments.
- Ensuring that all necessary documentation is completed accurately and submitted promptly.
- Maintaining communication with patients, families, and insurance companies to provide updates and address any concerns.
- Working alongside the clinical team to facilitate a smooth transition of care for new patients.

Requirements:
- Education: High school diploma or equivalent required. Preferred qualifications include a Licensed Vocational Nurse or an associate's degree in a healthcare-related field.
- Experience: A minimum of 2 years in a healthcare environment, ideally in intake coordination or patient services.
- Skills: Exceptional organizational and multitasking capabilities, along with strong written and verbal communication skills. Proficiency in electronic medical record systems, Slack, Microsoft Office, and Google Suite is essential.
- Knowledge: Understanding of home health services, medical terminology, and customer service principles.
- Ability to work both independently and collaboratively in a dynamic team setting.

The compensation for this role ranges from $25 to $34 per hour, based on experience.
