Job Summary

Executive Excellence is seeking a highly skilled Intake Coordinator to join our team in a substance abuse treatment program. The successful candidate will be responsible for conducting initial assessments for new patients, collecting data, and maintaining accurate patient records.

Key Responsibilities
  • Conduct one-on-one interviews with patients to collect data for the Addiction Severity Index (ASI) and Interpretive Summary
  • Enter data into the California Outcomes Measurements System (CalOMS) data collection system
  • Update patient files with urine drug screens, patient dosing records, and discharge information
  • Assist the front office manager with checking in patients, answering phone calls, and auditing charts for quality assurance
Requirements
  • Certification or registration with a California-approved substance abuse certification body (CCAPP, CADTP, or BBS)
  • Minimum six months experience working in a substance abuse treatment program
  • Experience with conducting patient intakes and basic computer skills
  • Good time management skills and ability to work independently
