Overview

As a vital member of the social services team, the role involves conducting comprehensive assessments to identify client needs, developing tailored service plans, and providing ongoing support to individuals and families.

Key Responsibilities

Engage with clients through interviews to collect essential information, assess their requirements, and formulate actionable service or case plans.

Act as a crucial link between clients, their families, law enforcement, judicial entities, and community organizations to facilitate coordinated care and develop effective prevention strategies.

Maintain accurate and up-to-date case documentation in accordance with established regulations and policies, ensuring timely reporting and compliance.

Provide individual, family, or group counseling sessions, leveraging community resources for follow-up and monitoring service provider compliance.

Assist colleagues as needed, which may include administering medications, transporting clients to appointments, and managing intake assessments or discharges.

Participate in team meetings to discuss client progress and share relevant information critical to service delivery.

Perform additional duties as assigned.

Qualifications

Education and experience equivalent to a Bachelor's degree in a relevant field such as legal, behavioral, or social sciences, along with one year of related work experience.

Skills and Competencies

Proficient in standard software applications and possess strong communication skills, both verbal and written, to foster effective relationships with colleagues and the public.

Must hold a valid driver's license with a clean driving record.

Special Requirements

This position necessitates the candidate to become a Certified Juvenile Probation Officer within six months of employment, passing a thorough background check, and undergoing mandatory drug testing.

Must be at least 21 years of age and meet all physical and written standards related to the role.

Work Environment

Standard office setting with the potential for travel to various locations. The role may involve working in a secure facility, requiring the ability to manage and restrain youth when necessary.
