Role Summary:
The Case Manager for Post Release Services (PRS) plays a crucial role in providing support to Unaccompanied Children (UC) and their families. This position may also involve conducting Home Studies (HS) as required. The Case Manager will assist families with resource referrals, community connections, and comprehensive case management services, all while employing trauma-informed care principles.

Key Responsibilities:
  • Deliver culturally and linguistically relevant Post Release Services (PRS) and thorough case management support;
  • Manage a caseload in alignment with contract stipulations and organizational expectations;
  • Conduct home visits as necessary to fulfill contract obligations;
  • Facilitate the reunification process through home study assessments when required;
  • Provide resource referrals and community connections while applying trauma-informed care throughout service delivery;
  • Conduct tailored assessments based on client needs in collaboration with the program supervisor;
  • Evaluate potential placements for unaccompanied children, including interviews and outreach to families in their home countries;
  • Document all necessary reports, case notes, and interactions in the designated database within specified timeframes;
  • Offer psycho-educational resources, referrals, and advocacy for children and families;
  • Assist unaccompanied children in accessing services through family stabilization counseling;
  • Establish and nurture positive relationships with community partners and resources;
  • Continuously assess the safety and suitability of placements using child welfare principles;
  • Serve clients statewide as assigned;
  • Participate in mandatory training and ongoing professional development;
  • Engage in weekly consultations with supervisors to review case plans;
  • Contribute to peer support initiatives for professional growth and case sharing;
  • Stay informed about community resources to assist families as needed;
  • Maintain awareness of agency, federal, and state regulations related to social services;
  • Complete additional tasks as assigned.
Qualifications:
  • Bachelor's degree in Human Services, Social Work, or a related field, with a minimum of two years of case management experience in child and family services, or three years of relevant experience in lieu of a degree;
  • Bilingual proficiency in English and Spanish;
  • Ability to work effectively with diverse client populations, including refugees and minority families;
  • Strong judgment and sensitivity to cultural differences and the unique experiences of refugees;
  • Knowledge of community resources available for victims of abuse;
  • Excellent verbal and written communication skills;
  • Demonstrated skills in clinical intervention and crisis management;
  • Capacity to work independently while maintaining confidentiality;
  • Proficient in Microsoft Office and other necessary computer skills;
  • Must be at least 21 years old with a valid driver's license and a clean driving record;
  • Pass a criminal history background check, including child protection agency registries;
  • Commitment to the agency's Statement of Faith and Mission Statement.
