Overview

As a Social Services Coordinator II, you will engage in essential social services activities, which include gathering information, assessing client needs, and formulating and executing service plans tailored to those needs. Your role will involve counseling clients and ensuring effective communication with their families and various service providers to address medical, social, financial, educational, housing, and related requirements.

Key Responsibilities

Conduct interviews with clients to collect necessary information, perform comprehensive needs assessments, and develop actionable service or case plans along with recommendations.

Act as a vital link between clients, their families, law enforcement, judicial systems, service providers, and community organizations to coordinate services, devise prevention strategies, and evaluate the effectiveness of the support provided.

Maintain meticulous case records in accordance with relevant regulations, policies, and procedures, ensuring timely and accurate reporting.

Provide individual, family, or group counseling, leveraging community resources for follow-up on referrals and overseeing service providers to ensure compliance with program standards.

Serve as a backup for team members, undertaking various responsibilities such as administering medications, transporting clients to appointments, conducting intake assessments, and managing client discharges.

Participate in team meetings to discuss client progress and share pertinent information critical to addressing client needs. Undertake additional duties as assigned.

Qualifications

Education and experience equivalent to a Bachelor's degree from an accredited institution in a legal, behavioral, or social sciences field, or a related area of study, along with one year of relevant work experience.

Essential Skills and Abilities

Proficient in standard software applications. Strong verbal and written communication skills, with the ability to establish and maintain effective working relationships with colleagues, departments, and the public.

Possession of a valid driver's license with a clean driving record is required.

Additional Requirements

This position necessitates certification as a Juvenile Probation Officer within six months of employment. Candidates must successfully pass a thorough background check and mandatory drug testing prior to employment, with ongoing random testing during employment.

  • Position involves working with juveniles who may exhibit aggressive behaviors; candidates should be prepared for an emotionally demanding work environment.
  • Employees may be reassigned to different departments, shifts, locations, or facilities as needed.
  • Working in a 24-hour facility may require mandatory overtime or extended shifts.
  • Employees may be classified as 'essential personnel' and may be required to remain on duty or return to work as necessary.
  • Applicants must be at least 21 years of age and must meet all physical and written standards of the Juvenile Academy to maintain employment.
Work Environment

Standard office environment with the requirement to travel to various work sites as needed.

This position may be situated within a secured facility and will require the ability to manage and restrain youth when necessary.
