Position Overview:
We are seeking a Director of Case Management for our esteemed client. This is a full-time permanent position that plays a crucial role in overseeing case management operations.
Key Responsibilities:
- Oversee and manage departmental functions, ensuring alignment with organizational policies and regulatory standards.
- Facilitate the integration of patient care and discharge planning processes across various departments.
- Lead interdisciplinary teams and engage with external stakeholders to enhance continuity of care.
- Ensure compliance with operational standards and drive performance improvements.
- Establish and maintain effective relationships with key stakeholders, including clinical and administrative leaders.
Qualifications:
- Bachelor's degree is required; a Master's degree in Nursing, Health Administration, or Business Administration is preferred.
- Must possess either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) credentials.
- A minimum of 3 years of experience in acute care hospital case management is preferred.
- At least 2 years of leadership experience in case management is preferred.
