Position Overview:
We are seeking a dedicated and experienced Director of Case Management to oversee and enhance our case management department. This role is critical in ensuring the delivery of high-quality patient care and effective discharge planning.
Key Responsibilities:
- Lead and manage departmental operations, ensuring compliance with organizational policies and regulatory standards.
- Coordinate with interdisciplinary teams to optimize patient care and resource utilization.
- Oversee daily case management functions and ensure achievement of performance metrics.
- Act as a registered nurse, providing operational leadership within the department.
- Utilize strong communication and leadership skills to motivate staff and drive departmental success.
- Implement change management strategies to enhance departmental initiatives and improve outcomes.
- Adapt to changing circumstances and manage multiple priorities effectively.
Qualifications:
- Bachelor's degree in a relevant field is required.
- A Master's degree in Nursing, Health Administration, or Business Administration is preferred.
- Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) is required.
- Minimum of 3 years of experience in acute care hospital case management is preferred.
- At least 2 years of leadership experience in case management is preferred.
