Job Title: HR Corrective Action Team Investigator

Job Summary:

The Corrective Action Team Investigator plays a crucial role in maintaining the quality of home health services by investigating incidents involving home health aides. This role involves assessing the validity of incidents, conducting thorough investigations, and recommending appropriate corrective actions, including counseling or disciplinary measures when necessary.

Key Responsibilities:

  • Conduct detailed investigations into incidents involving home health aides, ensuring a comprehensive understanding of the situation.
  • Gather and analyze relevant information, including statements from involved parties and any available documentation.
  • Assess the validity of incidents based on collected evidence and established policies and procedures.
  • Provide objective evaluations to determine the accuracy and seriousness of reported incidents.
  • Recommend and implement appropriate corrective actions, which may include counseling or disciplinary measures, based on the results of investigations.
  • Ensure that corrective actions align with organizational policies and are fair and consistent.
  • Maintain detailed and accurate records of incident investigations, including findings, recommendations, and actions taken.
  • Prepare comprehensive reports for management and relevant departments.
  • Collaborate with HR, management, and other relevant stakeholders to communicate investigation outcomes and coordinate any necessary actions.
  • Provide guidance and support to other departments in implementing corrective measures.
  • Participate in the development and improvement of policies and procedures related to incident reporting and corrective actions.
  • Identify trends and patterns in incidents to prevent reoccurrence and enhance overall quality of care.
  • Research, analyze and resolve conflict-related problems or questions.
  • Counsel caregivers on conflict issues.
  • Communicate with all departments and outside agencies to resolve conflicts.

Work Environment:

The Corrective Action Team Investigator works in an office environment with regular exposure to staff and patient elements and occasional stress.

Cognitive Requirements:

The Corrective Action Team Investigator must work cooperatively with others and perform a wide variety of complex and complete tasks involving office equipment and machinery.

Qualifications:

  • High school diploma or equivalent required; Additional education or certifications in a related field preferred.
  • Strong computer skills and proficiency in common office software applications.
  • Effective communication and interpersonal skills.
  • Detail-oriented and capable of working in a fast-paced environment.
  • Commitment to maintaining confidentiality and privacy standards.
  • Punctuality, reliability, and a team-oriented mindset.
