About the Role

Evergreen Treatment Services is seeking a highly motivated and compassionate Care Manager to join our REACH Program team. As a Care Manager, you will play a critical role in providing outreach, engagement, and intensive case management services to Spanish-speaking individuals experiencing homelessness and substance use disorders.

Key Responsibilities
  • Outreach and Engagement: Provide culturally sensitive outreach and engagement services to assigned clients, establishing trust and rapport through respectful and client-centered practices.
  • Case Management: Develop and implement individualized service plans in collaboration with clients, addressing their stated needs and priorities, and regularly reviewing and updating plans to reflect progress or attainment of goals.
  • Housing and Benefits Assistance: Assist clients in obtaining and maintaining housing stability, as well as accessing essential benefits and resources.
  • Health and Wellness: Assess clients' health and wellness needs, connecting them with primary medical care, addressing chronic health conditions, and facilitating engagement in behavioral health services.
  • Substance Use Disorder Treatment: Assess clients' substance use issues, matching clinical interventions to their stage of change, and facilitating engagement in treatment services.
  • Financial Stability: Assist clients in achieving financial stability through access to entitlement programs, employment opportunities, and financial planning.
  • Community Partnerships: Develop and maintain productive working relationships with community programs and services to ensure effective coordination of care.
Requirements
  • Education: High school diploma or equivalent required; undergraduate degree preferred.
  • Experience: Minimum two years' experience in social work or outreach programs, with experience providing services to individuals with substance use disorders from a harm reduction perspective.
  • Language Skills: Bilingual in Spanish and English, with the ability to read, write, and speak both languages fluently.
  • Knowledge and Skills: Computer literate with basic knowledge of Microsoft Office Suite, ability to prioritize workload, and excellent communication and interpersonal skills.
