Position Responsibilities
The Senior Case Manager will supervise the Case Management team, focusing on assisting Black men transitioning from chronic homelessness, many of whom may face significant mental or physical health challenges and/or substance use disorders. Our commitment is to provide a positive harm reduction approach, emphasizing a culturally responsive environment that meets individuals where they are, offering non-judgmental and non-coercive services and resources.
Key Duties:
- Clinical Responsibilities:
- Deliver culturally responsive cognitive behavioral therapy (CBT) to guests, both individually and in group settings, addressing emotional and mental health challenges.
- Conduct evaluations and assessments to facilitate referrals to treatment facilities and community resources tailored to specific guest needs.
- Establish and maintain strong relationships with clients in both group and individual contexts.
- Perform accurate clinical assessments, including mental and emotional health evaluations, and general physical assessments when necessary.
- Maintain thorough and precise records of each client's treatment journey.
- Engage in professional consultations with medical professionals and other relevant parties.
- Handle administrative tasks as required to coordinate with other social service organizations.
- Respond to crises effectively to de-escalate situations involving guests.
- Serve as a site preceptor for social work students as opportunities arise.
- Other duties as assigned.
- Team Leadership:
- Supervise four case managers responsible for guest screening, intake, data collection, and group facilitation.
- Ensure adequate staffing for a 24-hour enhanced living community.
- Assist in training and capacity building for case management staff to stay updated on best practices.
- Conduct regular meetings with case managers to review program goals and provide coaching.
- Maintain accurate records of direct reports' hours and ensure timesheets are completed accurately.
- Other duties as assigned.
- Case Management Oversight:
- Guide case managers in identifying barriers faced by community guests in achieving their goals.
- Build rapport and advocate for a diverse range of community residents, employing harm reduction strategies as needed.
- Participate in de-escalation and physical interventions during emergencies.
- Provide training on best practices for managing guest behaviors that may hinder program participation.
- Ensure case managers develop relationships with landlords and community resources to support guest success.
- Maintain current guest records and ensure accurate data entry.
- Other duties as assigned.
- Collaboration:
- Attend staff meetings, workshops, and training sessions as required.
- Contribute to a positive, team-oriented work environment and participate in group decision-making.
- Collaborate with program coordinators to align program activities with performance expectations.
- Work with the Development team to identify opportunities for program funding.

Working Conditions:
This role may involve exposure to blood, bodily fluids, and other potentially infectious materials, as well as cleaning supplies and chemicals. Working conditions may include interruptions and interactions with individuals in distress.
Minimum Qualifications:
- Licensed Clinical Social Worker with a master's degree in social work from an accredited institution.
- At least two years of clinical experience in a housing program or community-based healthcare setting.
- Experience providing culturally competent services to diverse populations, particularly Black men.
- Strong understanding of issues related to substance abuse, mental illness, trauma, and chronic homelessness.
- Knowledge of therapeutic options and treatments for trauma-affected individuals.
- Demonstrated empathy and emotional intelligence.
- Commitment to the mission and values of ACLT.
- Familiarity with local resources for individuals and families.
- Strong problem-solving and critical-thinking abilities.
- Excellent organizational and case management skills.
- Ability to work collaboratively within a team environment.
- Proficient in computer applications including Office 365 and electronic medical records.
- Willingness to learn and adapt in a diverse setting.
- Commitment to harm reduction principles.
- Experience in data collection and report writing.
Preferred Qualifications:
- Knowledge of local subsidized housing options.
- Experience working with chronically homeless adults.
- Proficiency in a second language.
- CPR/First Aid certification.
