Job Summary

We are seeking a highly skilled and compassionate Enhanced Service Coordinator to join our team at Presbyterian Villages of Michigan. As a key member of our staff, you will play a vital role in ensuring the well-being and independence of our residents.

Key Responsibilities
  • Conduct thorough assessments of resident needs and develop personalized plans to meet those needs.
  • Provide case management and referral services to residents, connecting them with community resources and services.
  • Coordinate communication between residents, families, medical staff, and administrative staff to ensure seamless care.
  • Develop and implement programs to promote resident wellness, independence, and social engagement.
  • Collaborate with local service agencies and providers to ensure high-quality services and support.
  • Establish and maintain relationships with residents, families, and community partners to build a strong support network.
  • Stay up-to-date on community resources, services, and regulations to ensure compliance and best practices.
Requirements
  • Bachelor's degree in a related field (e.g., social work, gerontology, healthcare administration).
  • 2-4 years of experience in a related field, preferably in senior living or healthcare.
  • Excellent communication, interpersonal, and organizational skills.
  • Proficiency in MS Office and ability to learn new software systems.
  • Knowledge of HUD regulations and Fair Housing laws.
