GENERAL OVERVIEW
Responsible for overseeing, coordinating, and executing direct services for clients while ensuring all documentation adheres to state and federal regulations relevant to HCS, TxHmL, and ICF-IID programs.

Collaborates within a service planning team to ensure the effective management of Medicaid waiver services, adopting a person-centered, trauma-informed, and individualized approach that employs evidence-based practices delivered with fidelity.

Services are provided across various environments, including field-based settings (clients' residences or designated locations) and governmental organizations that authorize the services provided.

KEY RESPONSIBILITIES
The essential functions outlined below represent the primary duties that an employee must be capable of performing, with or without reasonable accommodation:

  • Timely completion of all required documentation.
  • Active participation in Quality Assurance and Quality Improvement initiatives.
  • Ensure clinical documentation aligns with Center standards and fulfills all specified requirements.
  • Coordinate, implement, and monitor medical, social, educational, and other services as requested by clients.
  • Develop, implement, coordinate, and monitor PDP/IPC services for HCS and TxHmL clients.
  • Engage in client intake enrollment and transfer meetings.
  • Participate in Annual/IPC Service Planning Team (SPT) Meetings.
  • Document SPT meeting minutes, service justifications, goals, and training based on Annual/Revised PDPs and IPC revisions.
  • Facilitate SPT meetings to review assessments and services.
  • Conduct monthly Utilization Reviews and IPC revisions as necessary to adjust services.
  • Complete annual Implementation Plans in accordance with annual and/or revised PDPs and IPC.
  • Facilitate referrals for specialized therapies, including PT, OT, Speech, and Behavior Supports.
  • Coordinate clients' adaptive aids and minor home modifications, including liaising with DME companies and obtaining bids.
  • Conduct annual IDRC assessments for re-evaluation of diagnoses.
  • Perform face-to-face visits with clients to assess progress of IPC services.
  • Maintain communication with clients, legal guardians, and advocates to ensure continuity of care.
  • Complete ICAP assessments every three years or upon request for level change evaluations.
  • Review Host Home/Companion Care residential environments annually with assigned providers.
  • Ensure annual Special Needs training for relevant staff.
  • Coordinate Behavior Support Plan training for applicable staff.
  • Participate in Provider Monitor team meetings and state and federal surveys.
  • Collaborate with internal and external providers to meet client needs.
  • Facilitate crisis respite and Host Home placement situations.
  • Review Host Home logs and daily Respite logs for compliance with guidelines.
  • Ensure contractor compliance for services provided.
  • Prepare and correct invoices to reflect accurate service delivery.
  • Participate in contractor compliance reviews and facilitate necessary training.
  • Manage onboarding processes for contractors, including application reviews and training.
  • Submit contract requests to initiate services from onboarding contractors.
  • Review contractual agreements with onboarding contractors to clarify responsibilities.
  • Perform other related duties as assigned.

MINIMUM QUALIFICATIONS
Education: High School diploma or GED equivalency.
Experience: One (1) year of progressively responsible experience working with individuals with developmental disabilities.

OTHER REQUIREMENTS
Must possess a valid driver's license and automobile insurance, be able to travel as necessary, and meet the driving record requirements of the company's insurance provider.

Must maintain required credentials and training to comply with all state regulations and organizational policies.

Must be capable of meeting physical requirements for Nonviolent Crisis Intervention (NCI) and CPR training, including lifting and supporting specific weights, bending, and stooping.

PREFERRED QUALIFICATIONS
Bachelor's Degree in Social Work, Psychology, Counseling, or a related field, along with one (1) year of relevant experience.

Bilingual proficiency (English/Spanish) is preferred.

COMPETENCIES FOR SUCCESS
Knowledge of applicable software applications, direct consumer services, modern office procedures, and state regulations.

Ability to:
Effectively communicate, maintain accurate records, and uphold confidentiality.

This position requires light physical exertion, including the ability to lift and move objects within specified weight limits.
