GENERAL OVERVIEW
Responsible for overseeing, coordinating, and executing direct services for clients, ensuring all documentation aligns with state and federal regulations for HCS, TxHmL, and ICF-IID programs.

ROLE AND RESPONSIBILITIES
Works collaboratively within a service planning team to manage Medicaid waiver services for participants, employing a person-centered, trauma-informed, and individualized approach based on evidence-based practices.

Services are provided in various environments, including field-based settings such as clients' homes and authorized government locations.

KEY DUTIES INCLUDE:

  • Timely completion of all required documentation.
  • Participation in Quality Assurance and Quality Improvement initiatives.
  • Ensuring clinical documentation meets established standards and requirements.
  • Coordinating, implementing, and monitoring medical, social, educational, and other services as requested by clients.
  • Facilitating the development and monitoring of PDP/IPC services for HCS and TxHmL clients.
  • Engaging in consumer intake and enrollment meetings.
  • Participating in Annual/IPC Service Planning Team (SPT) Meetings and documenting meeting minutes, service justifications, and goals.
  • Conducting monthly Utilization Reviews and IPC revisions as necessary.
  • Completing annual Implementation Plans based on revised PDPs and IPCs.
  • Facilitating referrals for specialized therapies and coordinating adaptive aids and minor home modifications.
  • Conducting annual IDRC assessments and face-to-face visits with clients to evaluate IPC service progress.
  • Maintaining communication with clients, legal guardians, and advocates to ensure continuity of care.
  • Reviewing Host Home and Day Hab logs to ensure compliance with guidelines.
  • Participating in contractor compliance reviews and training sessions.
  • Coordinating crisis respite and Host Home placements.
  • Completing onboarding processes for contractors, including document reviews and training.

MINIMUM QUALIFICATIONS
High School diploma or GED and one year of relevant experience working with individuals with developmental disabilities.

OTHER REQUIREMENTS
Must possess a valid driver's license and automobile insurance, be able to travel as needed, and meet the driving record requirements of the company’s insurance provider.

PHYSICAL DEMANDS
The role requires light physical demands, including the ability to lift up to 20 lbs. occasionally and perform tasks that may involve bending and stooping.

WORK SCHEDULE
Standard working hours are Monday through Friday, 8 AM to 5 PM.
