GENERAL SUMMARY
Responsible for overseeing, coordinating, and implementing direct services for individuals with developmental disabilities, ensuring compliance with state and federal regulations associated with HCS, TxHmL, and ICF-IID programs.
Collaborates within a service planning team to provide comprehensive oversight of Medicaid waiver services, adopting a person-centered, trauma-informed, and individualized approach that utilizes evidence-based practices delivered with fidelity.
Services are provided in various environments, including field-based settings (such as clients' homes) and through governmental entities that authorize the services rendered.
ESSENTIAL DUTIES & RESPONSIBILITIES
The essential functions represent the core duties that an employee must be capable of performing, with or without reasonable accommodation.
The following tasks are indicative of the responsibilities associated with this role:
- Timely completion of all required documentation.
- Active participation in Quality Assurance and Quality Improvement initiatives.
- Ensuring clinical documentation adheres to Center standards and fulfills all designated requirements.
- Coordinating, implementing, and monitoring medical, social, educational, and other services as requested by clients.
- Developing, implementing, coordinating, and monitoring PDP/IPC services for HCS and TxHmL consumers.
- Engaging in consumer intake enrollment and transfer meetings.
- Participating in Annual/IPC Service Planning Team (SPT) Meetings.
- Documenting SPT meeting minutes, service justifications, goals, and training based on Annual/Revised PDPs and IPC changes.
- Facilitating SPT meetings to review assessments and services.
- Conducting monthly Utilization Reviews and IPC revisions as necessary to adjust services.
- Completing annual Implementation Plans in accordance with annual and/or revised PDPs and IPCs.
- Facilitating referrals for clients requiring specialized therapies (PT, OT, Speech, Behavior Supports).
- Coordinating adaptive aids and minor home modification purchases, including liaising with DME companies and obtaining bids.
- Conducting annual IDRC assessments for diagnosis re-evaluation.
- Performing face-to-face visits with clients to assess progress regarding IPC services.
- Maintaining communication with clients, legal guardians, and advocates to ensure continuity of care.
- Completing ICAP assessments every three years or upon request for level change evaluations.
- Reviewing Host Home/Companion Care residential environments annually with assigned providers.
- Ensuring annual Special Needs training for relevant staff.
- Coordinating Behavior Support Plan training as required.
- Participating in Provider Monitor team meetings and state and federal surveys.
- Collaborating with internal and external providers to meet client needs.
- Facilitating Utilization Reviews and participating in Death Reviews as needed.
- Reviewing all Host Home logs and daily Respite logs for compliance with guidelines.
- Overseeing contractor compliance for various services.
- Preparing invoices to accurately reflect services provided by contractors.
- Engaging in contractor compliance reviews and facilitating necessary training.
- Coordinating CPR training with the training department and contractors.
- Facilitating initial onboarding for contractors, including application reviews and training provision.
- Submitting contract requests for onboarding contractors.
- Reviewing contractual agreements with onboarding contractors for service expectations.
- Performing other related duties as required.
MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience: High School diploma or GED equivalency with at least one year of relevant experience working with individuals with developmental disabilities.
Licenses or Certifications: None required.
Other Requirements: Must maintain a valid driver's license and automobile insurance, be able to travel as necessary, and meet the driving record requirements of the company's auto insurance carrier.
Must uphold required credentials and mandatory training to ensure compliance with state regulations and organizational policies.
Must be capable of meeting physical requirements for Nonviolent Crisis Intervention (NCI) and CPR training, including lifting and supporting specified weights, bending, and getting on and off the floor independently.
PREFERRED QUALIFICATIONS
Bachelor's Degree in Social Work, Psychology, Counseling, or a related field, along with one year of relevant experience. Bilingual (English/Spanish) is preferred.
COMPETENCIES FOR SUCCESSFUL PERFORMANCE OF JOB DUTIES
Knowledge of applicable software applications, direct consumer services, modern office procedures, and state regulations.
Skills:
Strong organizational and time management abilities, proficiency in performing various duties, and basic mathematical functions.
Ability to:
Maintain organized records, communicate effectively, establish working relationships, and uphold confidentiality.

This position involves light physical demands, including the ability to exert force occasionally and frequently to move objects.
