Position Overview:
Join a dedicated team at BCFS Health and Human Services, where our mission is to positively impact the lives of individuals and families. We are seeking a full-time Care Coordinator to provide essential support and guidance to children and young adults facing complex challenges.

Academic Requirements:

  • Required: High school diploma or GED.
  • Preferred: Bachelor’s or Master’s degree in social work, psychology, criminal justice, sociology, or a related field.

Work Experience:

  • Required: For high school diploma holders, a minimum of four years of progressive experience working with children and/or young adults with complex trauma histories, including supervisory experience.
  • Preferred: For Bachelor’s and Master’s degree holders, at least two years of relevant experience, including progressive employment with children and/or young adults facing complex trauma.

Key Responsibilities:
1. Ensure compliance with all federal and state regulations relevant to this role.
2. Supervise staff to guarantee that assigned cases adhere to quality assurance standards, including accurate assessments and the development of service plans.
3. Identify training needs and collaborate with senior leadership to develop training programs.
4. Make informed decisions regarding the safety and well-being of children in consultation with the Program Director.
5. Engage with service providers and families involved in the child's casework.
6. Participate in community meetings to foster relationships and enhance program visibility.
7. Monitor team expenditures to ensure adherence to travel and safety protocols.
8. Review staff records and ensure compliance with agency standards and Care Coordination protocols.
9. Address performance challenges and ensure deadlines are met for all assigned cases.
10. Facilitate effective communication within the team and with external partners.

Additional Responsibilities:
1. Coordinate referrals and maintain documentation for the service population.
2. Act as a liaison with various service resources, including shelters, law enforcement, and medical providers.
3. Hire, train, supervise, and evaluate team members.
4. Meet all program deadlines and requirements.

Requirements:
1. Successfully pass pre-employment screenings and random drug tests.
2. Participate in all mandatory training sessions.
3. Provide proof of work eligibility when requested.
4. Clear pre-employment and biennial criminal background checks.

Essential Skills:
1. Ability to respond sensitively to the cultural and socio-economic characteristics of the service population.
2. Strong communication skills in English; bilingual ability in Spanish is highly preferred.
3. Capacity to work in a fast-paced environment while maintaining professionalism.
4. Proficiency in computer literacy relevant to the position.
5. Ability to work independently and collaboratively within a multidisciplinary team.
