About the Job:

We are seeking a skilled Case Manager to provide support services to clients in the Hartford, CT area. As a Case Manager, you will work closely with clients to identify their needs and provide assistance with securing basic needs and entitlements, such as Medicaid and housing subsidies.

Responsibilities:

  • Assist clients with daily tasks, such as budgeting, shopping, and bill-paying
  • Encourage social supports, such as peer activities and groups, and develop community connections
  • Help integrate mental health and addiction treatment and use of wellness services
  • Make connections with job services and employment alternatives
  • Provide ongoing Case Management activities to support clients in their success

Requirements:

  • Bachelor's Degree or equivalent experience
  • Bi-lingual Spanish preferred
  • Case Management experience in Supportive/Transitional Housing preferred
