The Youth & Family Services (YFS) division collaborates with parents and youth aiming to improve their well-being and mend their relationships. We partner with human services, child welfare systems, juvenile courts, and community service providers to ensure that individuals and families receive the necessary services and skills for healthy living.

We are currently seeking a full-time Intake Coordinator with a preference for bilingual candidates.

Compensation starts at $43,000, depending on experience and education.

This role involves conducting client intakes and screenings to determine suitability for program participation and facilitating referrals to additional services while maintaining comprehensive documentation. The Intake Coordinator will evaluate individuals referred for mental health and/or substance use issues, establish service objectives, and connect clients with educational institutions and community organizations to ensure optimal care and service coordination for addiction intervention, AODA counseling, and recovery support.

Key Responsibilities
  • Engage in discussions with referral sources and clinical supervisors to clarify the reasons for referral before program acceptance.
  • Facilitate orientation and intake processes to align clients with necessary services and resources, assisting them in reaching their service plan objectives.
  • Collaborate with clinical supervisors to assign cases appropriately.
  • Ensure accurate and timely documentation related to clients, including documentation, ECR finding letters, and status reports.
  • Conduct individual orientations to program services or educational sessions as required.
  • Provide referrals for clients' identified needs and offer crisis intervention when necessary.
  • Interview clients, review their records, and consult with other professionals to assess their eligibility for program participation.
  • Deliver specialized care coordination services to clients and their families addressing behavioral health concerns and other needs, including addiction and mental health.
  • Maintain a supportive relationship with clients through advocacy and resource assistance, covering areas such as medical needs, housing, vocational services, and benefits.
  • Conduct follow-up outreach to youth and families, which may include phone calls, texts, emails, and home visits.
Qualifications

The ideal candidate will possess:

  • A Bachelor's degree in human services, social work, psychology, or a related field, or two to four years of relevant experience.
  • Understanding of human behavior, including the assessment and identification of substance use disorders, particularly with special populations.
  • Familiarity with the Code of Ethics for substance abuse professionals.
  • Knowledge of federal drug and alcohol confidentiality laws (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act.
  • Awareness of the criminal justice system.
  • Proficiency in computer applications, including Microsoft Office and the Internet.
  • Ability to work effectively with diverse populations.
  • Flexibility to work varied hours and travel as required.
  • Capability to obtain CADC certification through the State Certification Board (IAODAPCA) within the first two years of employment, if mandated by the program.
  • A valid driver's license, current auto insurance, and reliable transportation are necessary for fulfilling job responsibilities.
