PHS operates the WholeYouNYC (WYNYC) initiative, a community resource network that fosters reliable connections between healthcare providers, health plans, and community organizations, ensuring access to essential resources that address social determinants of health. Our network comprises over 100 organizations offering diverse programs, such as food assistance, housing support, employment services, health insurance navigation, and sexual health resources, all aimed at promoting healthier lives and reducing health disparities.

Recently, PHS was designated as a regional lead entity for the Social Care Network (SCN) in Brooklyn, Manhattan, and Queens, in response to New York State's initiative to enhance social care delivery.

Program Focus:

The Healthcare-Community Partnerships (HCP) Capacity Building team leverages best practices from PHS's extensive experience in sexual and reproductive health to implement scalable healthcare capacity building projects. These initiatives aim to enhance health access and equity in New York by integrating health-related social needs (HRSN) screening and referrals into various healthcare settings, including hospitals, primary care, and behavioral health programs.

Role Summary:

We are seeking a skilled Training Manager to oversee the HCP Capacity Building team's training initiatives for SCN projects. This role will focus on enhancing the knowledge and skills of healthcare partner staff in implementing HRSN screening and ensuring that clients receive appropriate referrals to community resources. Key responsibilities include:

  • Managing the training portfolio, including scheduling, needs assessments, curriculum development, and training evaluations.
  • Supervising Practice Improvement Specialists, including recruitment and performance management.
  • Conducting training needs assessments to identify strengths and areas for growth among healthcare partners.
  • Maintaining and developing training materials and curricula to ensure relevance and effectiveness.
  • Monitoring training attendance and evaluating training effectiveness through participant feedback and surveys.
  • Collaborating with directors and specialists to facilitate learning sessions and provide technical assistance.
  • Preparing training-related reports and identifying opportunities for expanding training initiatives.
  • Traveling as necessary for meetings and project activities.

Qualifications:

  • Bachelor's degree in Public Health or a related field; Master's degree preferred.
  • 3-5 years of experience in health education or training.
  • Experience in staff supervision and training development.
  • Knowledge of community health systems and provider networks.
  • Background in healthcare, social services, or health equity preferred.

  • Exceptional facilitation and communication skills, with the ability to engage diverse audiences.
  • Strong organizational and project management abilities.
  • Proficiency in Microsoft Office and web-based meeting platforms.
