PHS operates the WholeYouNYC (WYNYC) initiative, a comprehensive community resource network that establishes reliable connections between healthcare providers, health plans, and community organizations. This network, comprising over 100 organizations, offers essential programs such as food assistance, housing support, employment services, health insurance navigation, and sexual health resources across all five boroughs. Our efforts aim to enable New Yorkers to achieve optimal health and reduce health disparities, contributing to the advancement of health equity.
Recently, New York State announced a significant funding opportunity to support Social Care Network (SCN) lead entities tasked with coordinating social care delivery throughout the state. PHS, along with the WYNYC network, has been designated as the regional SCN for Brooklyn, Manhattan, and Queens.
Program Overview:
The Healthcare-Community Partnerships (HCP) Capacity Building team leverages best practices and innovative strategies derived from PHS's extensive experience in sexual and reproductive health and healthcare-community collaborations. The team is dedicated to executing scalable healthcare capacity building initiatives that align with current healthcare reforms aimed at enhancing health access and equity in New York. The HCP Capacity Building team will facilitate the integration of HRSN screening and referrals to community resources across various healthcare settings within PHS's SCN, including hospitals, primary care, sexual and reproductive health services, behavioral health, and care management programs.
Position Responsibilities:
We are seeking a qualified Program Manager to oversee the management, administration, and execution of daily operations for one or more Quality Improvement Learning Collaboratives (QILCs). These collaboratives are designed to assist healthcare partners in implementing HRSN screening using established tools, referring to PHS's SCN, and ensuring that identified HRSN needs are adequately addressed. Reporting to the Director of HCP Capacity Building, the Program Manager will handle project administration, monitor programmatic and evaluation progress, facilitate internal communication and reporting, resolve issues impacting project timelines, and assist in organizing learning sessions, webinars, and training events.
Key Responsibilities Include:
- Overseeing project workplan activities, internal reporting, quality assurance, compliance, and evaluation in collaboration with the Director and HCP Capacity Building team.
- Coordinating healthcare partner submissions of project deliverables, including quality improvement tools and data reports, and following up as necessary.
- Managing preparations for project learning sessions and technical assistance calls, including logistics, material preparation, and coordination with partner sites.
- Facilitating or co-facilitating internal and external meetings or events as assigned.
- Supporting internal reporting on key project milestones, challenges, outcomes, and performance metrics.
- Acting as the primary contact for external partners regarding administrative aspects of SCN capacity building initiatives.
- Staying informed on research, public policy, and best practices related to HRSN screening and service provision networks.
- Traveling as needed for meetings and project activities.
Qualifications:
- Bachelor's degree in Public Health or a related field; a Master's degree is preferred.
- 3-5 years of experience managing healthcare or social service programs/projects.
- Experience with continuous quality improvement processes in healthcare settings is preferred.
- Knowledge of community health systems and provider networks in New York City is advantageous.
