Overview: Under the general guidance of the HIV Prevention Program Provider/Director and in collaboration with the Medical Services Director, this role involves community outreach, education, and case management services aimed at at-risk African American and Latinx populations residing in South Los Angeles. The ideal candidate will possess knowledge of the South Los Angeles area and demonstrate proficiency in engaging with at-risk groups, fostering education, raising awareness, recruiting, and enrolling eligible individuals into HIV prevention programs. Responsibilities include attending provider and community meetings to enhance awareness and establish referral networks with other organizations. This position also entails conducting rapid HIV testing and counseling, facilitating group sessions, and making referrals to HIV care as necessary. Prior experience in social services, health, or substance abuse settings is highly desirable.
  • Deliver personalized and confidential risk assessments and medication adherence evaluations to support clients in reducing risks and improving adherence.
  • Develop resources for PrEP/PEP services for clients in need and establish a network with PrEP/PEP providers.
  • Ensure access to HIV prevention methods, including biomedical prevention services (PrEP/PEP), and facilitate follow-up and adherence to these services, along with supportive measures that promote risk reduction and behavior change.
  • Facilitate access and referrals for STI/STD screenings.
  • Collaborate with project staff to promote SCFHC's HIV prevention and treatment initiatives.
  • Conduct rapid HIV testing and counseling at various community venues, including mobile units and health fairs as required.
  • Follow up on referrals to community services to ensure appropriate support.
  • Guide individuals diagnosed with HIV towards case management, medical care, and other necessary services, ensuring thorough follow-up and documentation of all referrals.
  • Provide accurate information regarding HIV transmission, prevention, and testing to the community.
Qualifications:
  • Bachelor's Degree in a health-related field or equivalent years of relevant educational/work experience.
  • Experience in substance abuse, human services, or social service environments is preferred.
  • State certification in HIV testing and counseling is advantageous.
  • Completion of the American Academy of HIV Medicine non-provider curriculum within six months of hire is required.
  • Completion of National STI/STD & HIV Curriculum within six months of hire is required.
  • Two to four years of experience in providing human services, health care services, or outreach services.
  • Proficiency in data entry and collection, with the ability to present information in both graphic and oral formats.
  • Formal training in HIV education and prevention is highly desirable.
  • Valid California Class "C" driver's license is necessary.
  • Proficient in Microsoft Office products (Word/Excel), communication systems, and internet search capabilities.
  • Bilingual and bi-literate in Spanish/English is preferred.
