Position Objective:
The role of the Housing Support Specialist is to provide assistance to individuals facing the threat of homelessness, including those who are chronically homeless or belong to homeless families. As a vital member of a mobile support unit, this position emphasizes client advocacy, outreach, and case management, while facilitating access to essential benefits. The specialist serves as a key connection between clients and mental health or substance abuse services, ensuring they receive stable housing and other critical supportive resources. By leveraging their expertise, they empower individuals to navigate and overcome the unique challenges encountered on their journey to achieving and maintaining stable housing and overall wellness.
Main Responsibilities:
- Engage in outreach efforts by collaborating with the mobile support team to connect with and build relationships with individuals experiencing homelessness in various locations.
- Establish trust and rapport to effectively understand the needs and aspirations of clients.
- Conduct comprehensive initial assessments and create intervention plans that address housing needs, service requirements, crisis management, and continuity of care.
- Deliver case management and navigation services aimed at assisting clients and their families in securing and maintaining stable housing.
- Support clients in completing housing applications, managing supportive and subsidized housing documentation, researching affordable housing options, and advocating with potential landlords.
- Collaborate with other organizations, coalitions, and community meetings to enhance service delivery.
Qualifications:
- Associate's or Bachelor's Degree in Social Work, Psychology, Counseling, or a related discipline.
- 1 to 2 years of experience in homeless services or case management, including housing navigation or community health work.
- Familiarity with housing resources, HUD guidelines, and various housing programs.
- Excellent communication and relationship-building skills.
- Proficient in Microsoft Word, Excel, and PowerPoint.
- Understanding of mental health and substance abuse services.
- Strong crisis intervention and problem-solving skills.
- Ability to adapt and remain flexible in dynamic environments.
