Job Summary

We are seeking a skilled Licensed Master Social Worker to join our team at Prisma Health. As a key member of our Emergency Department interdisciplinary team, you will provide emergency services within your scope of practice, specializing in assessments and interventions for patients with acute behavioral health, medical, and psychosocial emergencies.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments of patients' prior functioning levels, support systems, and reaction to illness, identifying and referring them to community resources as needed.
  • Collaborate with Emergency Medicine providers, psychiatrists, and the interdisciplinary team to assess patients' behavioral health and substance abuse needs, referring them to community resources and coordinating transfers to IP psychiatric hospitals/substance abuse treatment centers when indicated.
  • Coordinate and facilitate tele-psychiatry consults for appropriate patients, serving as an extension of community mental health centers by coordinating mental health pick-up orders with local probate judges when mental health centers are closed.
  • Manage oversight and compliance of emergency psychiatric/chemical dependency hospitalization papers in accordance with state law, assisting Emergency Medicine Providers in completing Inability to Consent documentation by identifying the appropriate surrogate decision maker in compliance with the Adult Healthcare Consent Act.
  • Coordinate referrals for post-hospitalization services/needs, including skilled and long-term nursing facilities, assisted living, group homes, homeless shelters, new dialysis, home health, hospice, palliative care, durable medical equipment, infusion services, medication assistance, transportation needs, financial assistance, medical legal partnership, and other community programs/services.
  • Provide intensive case management services for ED high utilizers in conjunction with EM physicians and community partners, collaborating with EM physicians in developing care plans for this population and advocating for patient, family empowerment, and independence to make autonomous healthcare decisions and access needed services within the healthcare system.

Requirements

A Master's Degree in Social Work is required, and licensure as a Licensed Master Social Worker (LMSW) or a Licensed Independent Social Worker (LISW) by the South Carolina Board of Social Work Examiners is necessary. One year of social work experience, preferably in a healthcare setting, is preferred, as is certification in ACM, CCM, C-SWCM, or ACSW.
