About This Role

We are seeking a passionate and dedicated Community Health Worker (CHW) Program Coordinator to join our team. In this role, you will play a vital part in developing, implementing, and managing programs that empower individuals and communities to improve their health outcomes.

Key Responsibilities

  • Lead the development and implementation of innovative CHW-centered initiatives aligned with statewide, regional, and national health priorities.
  • Collaborate with diverse stakeholders, including community organizations, healthcare providers, policymakers, and researchers, to ensure program effectiveness and impact.
  • Provide guidance and support to CHWs, fostering their professional development and growth within the field.
  • Oversee program evaluation and data collection, analyzing results to inform program improvements and demonstrate impact.
  • Advocate for policies and practices that promote health equity and address social determinants of health.

Essential Functions

This role requires a high level of engagement in community outreach, collaboration, and program management. You will be responsible for:

  • Building strong relationships with community partners and stakeholders.
  • Developing and delivering training programs for CHWs.
  • Conducting needs assessments and program evaluations.
  • Preparing reports and presentations to share program outcomes and recommendations.

Qualifications

We are seeking a candidate with:

  • A Bachelor's degree in public health, social work, or a related field.
  • Experience working with community-based organizations and/or CHWs.
  • Strong communication, interpersonal, and organizational skills.
  • A commitment to health equity and social justice.
