Position Summary

The Community Social Worker (CSW) is an LCSW or LCSW-C and is part of the Hospital's

Community Care Team (CCT). CCT is a program focused on improving the health of complex,

chronically ill Howard County residents. This program helps individuals connect with resources

that allow them to better manage their illness, improve their health and avoid preventable

hospital encounters. CCT helps clients to meet their goals by providing health education,

teaching chronic-disease management skills, and by connecting them to a medical home and

other community resources. The CSW is an integral part of the multi-disciplinary team that

includes Community Health Nurses (CHN) and Community Health Workers (CHW).

This position is responsible for conducting comprehensive assessments of CCT patients'

psychosocial needs, provides client education, advocacy, counseling and support and assists

clients with connecting to behavioral health treatment. This position maintains a caseload in

addition to working with CHNs and CHWs to determine appropriate levels of emergent

behavioral health services as needed. The CSW supervises the CHW embedded in the

Emergency Department.

MAJOR JOB RESPONSIBILITIES:

Working closely with the CCT Program Manager, principal duties include the following:

1. Conducts comprehensive assessment of clients' psychosocial needs

2. Performs home visits as needed

3. Provides client education, advocacy, counseling and support

4. Manage assigned caseload of clients

5. Assists clients with connecting to behavioral health treatment

6. Assists Community Health Nurses and Community Health Workers with triaging and

understanding patient's psychosocial needs and determining appropriate levels of emergent

care services as needed.

7. Maintains extensive knowledge about community resources appropriate to needs of clients

8. Works closely with medical provider(s) and other members of the patients' care team to

provide progress updates and help ensure comprehensive and coordinated care.

9. Completes timely and accurate documentation of program activities

10. Provides direct supervision of CHW embedded in the Emergency Department
