Job Summary

PruittHealth is seeking a skilled Social Worker to join our team in providing compassionate care to patients and their families. As a Social Worker, you will play a vital role in assessing patients' needs, developing care plans, and coordinating services to ensure the best possible outcomes.

Key Responsibilities
  • Conduct thorough assessments of patients' social, emotional, and environmental needs
  • Develop and implement individualized care plans to address patients' needs and goals
  • Coordinate with healthcare teams to ensure seamless care delivery
  • Provide emotional support and counseling to patients and their families
  • Collaborate with community resources to ensure patients receive necessary services
Requirements
  • Master's Degree in Social Work (MSW) from an accredited program
  • One year of experience in social work, preferably in a healthcare setting
  • Current licensure as a Master's level social worker
  • Ability to work a flexible schedule, including evenings and weekends
  • Excellent communication and interpersonal skills
Preferred Qualifications
  • Orientation and training to complete assigned duties effectively
  • Ability to work independently and as part of a team
  • Positive criminal history check and licensure validations
  • Attendance and punctuality
