Position Overview:

The role of the Intake and Assessment Therapist is crucial in delivering comprehensive clinical services and administrative support to clients. This position encompasses a variety of responsibilities aimed at ensuring effective client engagement and assessment.

  1. Informed Consent Acquisition
  • Facilitate the acceptance of client Informed Consent documents through the patient portal.
  • Verify that all documentation is accurately completed before initiating the Intake Assessment.
  • Conduct outreach to offer rescheduling options for clients who have not completed their paperwork.
  1. Client Assessment and Diagnosis
  • Ensure each client completes necessary assessments, including the PHQ and Columbia-Suicide Severity Rating Scale during the Intake Assessment.
  • Collect relevant clinical information to formulate an informed preliminary diagnosis.
  • Support all diagnoses with justification based on DSMV criteria.
  • Evaluate the client's level of care and facilitate referrals to higher levels of care when necessary.
  1. Clinical Documentation
  • Complete all required documentation, including Biopsychosocial assessments and missed appointment notes, within 48 hours of service delivery.
  • Maintain precise and thorough records of client interactions.
  1. Clinical Recommendations and Scheduling
  • Provide clinically justified recommendations for the client's treatment plan.
  • Educate clients about recommended treatment services.
  • Assist in linking clients to suggested services such as therapy, medication management, and specialty services.
  1. Crisis Intervention
  • Assess the client's current risk level effectively.
  • Ensure client safety through appropriate crisis intervention strategies.

Key Responsibilities:

  • Review and accept client Informed Consent documents.
  • Ensure proper electronic signatures are obtained for all Informed Consent.
  • Deliver direct service by assessing clients' bio-psycho-social needs within a designated session timeframe.
  • Participate in monthly supervision with the Intake and Assessment Supervisor.
  • Engage in weekly Intake Clinical Meetings.
  • Facilitate referrals for clients requiring higher levels of care.
  • Meet Key Performance Indicator expectations.
  • Respond to internal communications in a timely manner.

Performance Metrics:

The performance of Intake and Assessment Clinicians will be evaluated based on several key competencies, including but not limited to:

  • 70% of admitted clients successfully linked to therapeutic services.
  • 10% of admitted clients connected to specialty services.
  • Achieving an average client satisfaction rating of 4.3 out of 5 or higher.
  • Completion of documentation within 48 hours of service.
  • Chart audit scores maintained at or above 80% each quarter.
  • Adherence to processes related to higher level of care recommendations and high-risk reporting.

Job Type: Full-time

Required Education: A master's degree in a Human Services field and Full or Associate Licensure in the state of practice.

Required Experience: A minimum of 2 years of experience in a Mental Health Treatment Facility.
