Role Summary

Hartford Healthcare is actively seeking a Nurse Practitioner or Physician Assistant to join a well-respected, collaborative Transplant Clinic team. The Advanced Practice Provider (APP) will primarily engage with patients in an outpatient environment.

Position Type: Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA-C)

Work Location: Outpatient facilities within Hartford

Work Environment: Outpatient clinic responsibilities

Work Schedule: 40 hours weekly; occasional weekends and holidays may be necessary

Key Responsibilities:

· The Outpatient Post-Transplant Advanced Practice Provider will deliver comprehensive healthcare services to patients who have received kidney or other solid organ transplants.

· The role involves managing post-transplant care, which includes monitoring patient recovery, overseeing immunosuppressive treatment, and coordinating ongoing care.

· Conduct outpatient visits for post-transplant patients, including gathering medical histories, performing physical assessments, and ordering necessary diagnostic tests.

· Address both acute and chronic post-transplant challenges, such as infection prevention, rejection surveillance, and medication side effects.

· Collaborate with transplant surgeons, nephrologists, nursing staff, social workers, and other healthcare professionals to ensure comprehensive patient management.

· Coordinate follow-up care, which includes scheduling appointments, managing referrals, and liaising with other specialists.

· Maintain precise and current patient records, including progress notes, treatment strategies, and communications with other healthcare providers.

· Ensure all documentation complies with regulatory standards and institutional policies. Engage in quality improvement initiatives and contribute to the development of best practices for post-transplant care. Stay informed about advancements in transplant medicine through ongoing education and professional development opportunities.

Qualifications

· Licensure: Unrestricted State of Connecticut Medical License (or eligibility for licensure)

· Education: APRN (Family, Pediatric, Adult/Geri Nurse Practitioner) or PA

· Experience: A minimum of two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) is preferred but not mandatory.
