About the Role

Hartford Healthcare is looking for a skilled Nurse Practitioner or Physician Assistant to join our esteemed and collaborative Transplant Clinic. The Advanced Practice Provider (APP) will be responsible for delivering care to patients in an outpatient environment.

Role Specifications:

This position is open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA-C).

Location: Outpatient facilities are situated in Hartford.

Work Environment: Focused on outpatient responsibilities within the clinic.

Work Schedule: 40 hours per week; occasional weekends and holidays may be necessary.

Scope of Practice for Nurse Practitioners and Physician Assistants:

  • The Outpatient Post-Transplant Practitioner will provide comprehensive medical services for individuals who have received kidney or other solid organ transplants.
  • Responsibilities include managing post-transplant care, monitoring patient progress, overseeing immunosuppressive therapy, and coordinating follow-up care.
  • Conduct outpatient clinic visits for post-transplant patients, which involves taking medical histories, performing physical examinations, and ordering necessary diagnostic tests.
  • Address both acute and chronic post-transplant concerns, including infection prevention, rejection monitoring, and managing medication side effects.
  • Collaborate with transplant surgeons, nephrologists, nurses, social workers, and other healthcare professionals to ensure holistic patient care.
  • Coordinate follow-up care, which includes scheduling appointments, managing referrals, and liaising with other specialists.
  • Maintain precise and current patient records, including progress notes, treatment plans, and communications with other healthcare providers.
  • Ensure all documentation adheres to regulatory standards and institutional policies. Participate in quality improvement initiatives and contribute to the development of best practices for post-transplant care. Stay informed about advancements in transplant medicine through ongoing education and professional development opportunities.

Qualifications

  • Licensure: Unrestricted State of Connecticut Medical License (or eligibility)
  • Training: APRN (Family, Pediatric, Adult/Geri Nurse Practitioner) or PA
  • Experience: A minimum of two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) is preferred but not mandatory.
