Position Overview:

As a vital member of a collaborative healthcare team, the Advanced Practice Nurse Specialist is responsible for delivering comprehensive clinical services. This role encompasses client care responsibilities, including professional assessment, diagnosis, and the prescribing and administration of treatments. Utilizing extensive medical expertise, the practitioner engages in the diagnosis, prevention, and management of various health conditions, contributing to the development of treatment plans alongside a multidisciplinary team comprising case managers, therapists, nurses, and program managers.

Additional responsibilities include providing consultation on diagnoses and treatment options to other healthcare professionals and community partners, as well as participating in the evaluation of medical necessity and other utilization management activities.


Key Responsibilities:

- Demonstrate the ability to work autonomously when necessary while collaborating effectively with other healthcare providers to conduct thorough age-appropriate assessments for patients with complex acute, critical, and chronic health issues.

- Order and interpret diagnostic and therapeutic tests tailored to the specific needs of patients across different age groups.

- Prescribe suitable pharmacological and non-pharmacological treatment options.

- Implement interventions aimed at helping patients achieve or maintain physiological stability.

- Monitor the effectiveness of treatment plans through appropriate follow-up with patients.

- Perform various medical procedures as required, based on competency, including administering injections, assisting with minor surgical procedures, obtaining necessary cultures, and conducting EKGs and phlebotomy.

- Collaborate with team members to ensure appropriate referrals are made.

- Support decision-making for staff, patients, and families by providing educational resources.

- Carry out additional duties as assigned by management.



Qualifications:
Knowledge, Skills, and Abilities:
- Proficient in computer applications, including Word, Excel, and PowerPoint.

- Demonstrated clinical expertise and comprehensive knowledge in cardiovascular nursing.

- Proven ability to establish and maintain effective communication with nursing staff and other healthcare professionals, including the capacity to address large groups.

- Must possess and uphold knowledge and competency in providing care for diverse patient populations, including adolescents and adults.



Educational and Professional Requirements:
- Must be a Registered Nurse in the State of CT, holding a Master's Degree in Nursing.

- Preferred: Two years of experience in an outpatient care environment.



Certifications and Licenses:
- Board Certification as an Advanced Practice Registered Nurse (APRN).

- Current Nurse Practitioner certification in the relevant specialty area.

- Current CNOR/CRNFA certification (if applicable).

- Current Basic Life Support (BLS) provider card; CPR certification required.
