About the Role
Hartford Healthcare is actively seeking a Nurse Practitioner or Physician Assistant to join our esteemed and collaborative Transplant Clinic. The selected candidate will engage with patients in an outpatient environment.
Role Specifications:
We welcome applications from Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA-C).
Location: Outpatient Offices situated in Hartford.
Clinical Setting: Outpatient responsibilities within a clinic.
Work Schedule: 40 hours weekly; occasional weekends and holidays may be required.
Responsibilities of the Nurse Practitioner and Physician Assistant:
- Provide comprehensive medical care for patients who have received kidney or other solid organ transplants.
- Manage post-transplant care, including monitoring patient progress and overseeing immunosuppressive therapy.
- Conduct outpatient clinic visits, including gathering medical histories, performing physical examinations, and ordering necessary diagnostic tests.
- Address both acute and chronic post-transplant concerns, such as infection prevention and rejection monitoring.
- Collaborate with a multidisciplinary team, including transplant surgeons, nephrologists, nurses, and social workers, to ensure holistic patient care.
- Coordinate follow-up care, including appointment scheduling and managing referrals.
- Maintain accurate patient records, including progress notes and treatment plans, ensuring compliance with regulatory standards.
- Engage in quality improvement initiatives and contribute to the development of best practices in post-transplant care.
- Stay informed on advancements in transplant medicine through ongoing education and professional development.
Qualifications
- Licensure: Unrestricted State of Connecticut Medical License (or eligibility).
- Training: APRN (Family, Pediatric, Adult/Geri Nurse Practitioner) or PA.
- Experience: A minimum of two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA) is preferred but not mandatory.
