Position Overview

Sea Mar Community Health Centers is a community-focused organization dedicated to delivering high-quality, comprehensive health and social services to a diverse population, particularly serving the Latino community in Washington State. We are currently seeking a qualified individual for the following role:

Case Manager for Behavioral Health Housing

Hourly Compensation: $22.18

Role Summary:

We are looking for a Full-time Case Manager to join our expanding Behavioral Health Housing Program based in Vancouver, WA. This role requires a proactive and collaborative approach to support clients in navigating housing resources and implementing effective housing interventions.

Key Responsibilities:

  • Engage in outreach initiatives to connect with potential clients and coordinate efforts with external organizations.
  • Provide peer support and counseling, leveraging shared experiences to empower clients in their recovery journey.
  • Act as a mentor, fostering hope and self-advocacy among clients.
  • Conduct ongoing assessments to evaluate the psychological, social, vocational, and economic needs of clients.
  • Manage documentation and reporting for the PATH program, while assisting clients in securing housing solutions.

Additional responsibilities may be assigned as necessary.

Qualifications:

  • A Bachelor's degree in Counseling, Social Work, Psychology, or a related field is preferred; relevant work experience may be considered in lieu of a degree.
  • Must be an Agency Affiliated Counselor in Washington State.
  • Possession of an active Certified Peer Support Certification in Washington State is required.
  • Access to a vehicle, a valid driver's license, and current auto registration are necessary.
  • Must authorize Sea Mar to obtain a current driving record and maintain eligibility based on insurance standards.
  • Proof of auto insurance must be provided upon hire and at renewal.
  • Must pass a TB test upon hire and annually thereafter.
  • Basic Life Support (BLS) CPR certification is required within 90 days of hire and must be maintained throughout employment.
