Job Summary

We are seeking a skilled Schedule Specialist to join our team at Commonwealth Home Health, a part of LHC Group. As a Schedule Specialist, you will play a critical role in ensuring the efficient scheduling of visits with field staff, taking into account business needs, patients' needs, and employee needs.

Key Responsibilities
  • Utilize EMR and scheduling tools to schedule visits, ensuring timely and effective coordination of care.
  • Process workflow for scheduled, missed, rescheduled, reassigned, declined, and delivered visits.
  • Monitor Pending Referrals daily and assign licensed professionals and patient care managers for all SOC visits.
  • Communicate daily with field staff regarding unaddressed visits in late, pending, or incomplete status.
  • Run Scheduling Request Report daily and complete requests as necessary for Medicare week.
  • Monitor productivity and distribution of regular visits for caregiver optimization.
  • Review Unverified Visit report daily and communicate with field staff, patient care managers, and clinical directors as needed.
  • Process assigned tasks, coordination notes, and administrative tasks in a timely manner.
  • Assist in coordination of scheduling and tracking supervisory visits and therapy reassessment visits, as required by regulatory requirements.
Requirements
  • High school education or equivalent.
  • Minimum one year of scheduling experience in a healthcare setting using an online scheduling system.
  • Exceptional organizational, customer service, communication, and decision-making skills.
  • Working knowledge of state and federal regulations governing OASIS visits, supervisory, and reassessment visits.
