Key Responsibilities
  • Conduct comprehensive assessments of patients to develop personalized care plans
  • Provide and coordinate patient care in accordance with physician orders and agency policies
  • Collaborate with other healthcare professionals to ensure continuity of care
  • Document patient information accurately and timely
  • Ensure patients meet home health eligibility and medical necessity guidelines
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
