Housing Stability Case Manager

KEY RESPONSIBILITIES
The Homebase Program is recognized as NYC's leading initiative for preventing homelessness.

This community-driven program supports families facing the risk of entering shelters by addressing their housing challenges and fostering self-sufficiency.

The Case Manager role within the Homebase Program entails delivering comprehensive housing support and case management services. The individual will provide continuous social services to participants, aiming for sustainable housing solutions.

Responsibilities Include:
Facilitating services and resources to equip participants with the essential tools for maintaining their housing and avoiding homelessness;

Collaborating with clients, their families, supervisors, and additional staff to implement the service plan;

Setting objectives and actions that tackle both immediate housing issues and their root causes;

Tracking and assessing the progress of participants in executing their service plans;

Discussing client cases with supervisors and program directors to determine the need for financial assistance;

Preparing necessary documentation for in-house subsidy applications on behalf of clients;

Providing referrals to clients for accessing community resources, public benefits, and entitlements;

Maintaining precise records through case files and the designated database;

Coordinating, monitoring, and delivering services to Homebase program participants;

Ensuring the protection of participants' rights;

Performing additional duties as assigned by supervisors or program directors.

QUALIFICATIONS
Bilingual proficiency in English and Spanish is preferred.

Experience in non-profit sectors and a commitment to social equity.

Strong communication skills with an ability to advocate effectively.

Capacity to work autonomously while also collaborating within a team.

Entrepreneurial mindset with problem-solving abilities and relationship-building skills.

EDUCATIONAL BACKGROUND
An Associate Degree is required, along with three years of relevant experience; a Bachelor's degree is preferred.
