Overview of Supportive Housing Services: CAMBA's Supportive Housing Services aim to deliver essential social services to tenants residing in designated housing developments. This initiative encompasses a substantial number of units that will benefit from case management and additional supportive services.

Role: Case Coordinator

Reporting Structure: Reports to the Program Manager

Key Responsibilities:

  • Evaluate documentation to confirm tenant eligibility for the program and maintain organized records.
  • Establish and manage comprehensive tenant files.
  • Conduct initial assessments and ongoing evaluations of tenants and their families' needs.
  • Collaborate with tenants to develop and revise independent living plans, setting both short-term and long-term objectives.
  • Assist tenants in achieving their goals by identifying community resources and making appropriate referrals.
  • Support tenants in overcoming obstacles to their goals and encourage self-advocacy towards self-sufficiency.
  • Propose and implement strategies to enhance tenant engagement in the process.
  • Monitor tenant progress through regular communication and document findings accordingly.
  • Participate in case conferences to discuss tenant needs and progress.
  • Follow up with tenants and referral organizations to ensure effective communication and support.
  • Compile and provide necessary information for reporting purposes.
  • Recommend case closures when tenants have met their primary goals or are no longer eligible for services.
  • Act as a liaison for tenants with external organizations concerning education, healthcare, and legal matters.
  • Assist tenants in scheduling appointments and may accompany them as needed.
  • Support tenants in completing applications for benefits and entitlements.
  • Conduct follow-up interactions post-goal achievement to ensure tenant stability.
  • Evaluate living conditions through home visits when necessary.
  • Contribute to the development of marketing materials for the program.
  • Pre-screen tenants for eligibility and arrange intake appointments.
  • Input tenant data into the automated database for tracking progress.

Qualifications:

  • A Bachelor's degree (e.g., B.A., B.S.W.) and experience working with the Seriously and Persistently Mentally Ill (SPMI) population, or equivalent experience.

Additional Requirements:

  • Must undergo fingerprinting and obtain Criminal History Checks (CHC).
  • Ability to maintain fingerprint clearance throughout employment.
  • As a Mandated Reporter, immediate reporting of suspected abuse or neglect is required.
  • Flexibility in working hours may be necessary to provide coverage as needed.

Compensation: $40,000-$45,000 annually.
Salary ranges reflect the low and high end for the position and are determined by various factors including experience and education.

Employment Status: Full-time (35 hours per week)
