KEY RESPONSIBILITIES
The Homebase Program stands as New York City's leading initiative for preventing homelessness.

This community-focused program aids households facing the risk of entering shelters by addressing their housing emergencies and fostering pathways to self-sufficiency.

The Case Manager role within the Homebase Program entails delivering comprehensive housing support and integrated case management services. The individual will offer continuous social services to participants, aiming to achieve enduring housing stability.

The Case Manager will:
Facilitate services and activities that equip program participants with essential resources to maintain their housing and avert homelessness.

Collaborate with clients, their family members, supervisors, and additional staff to implement the service plan effectively.

Set objectives and actions that tackle both the immediate housing emergency and its root causes.

Track and assess the progress of program participants in executing their service plans.

Engage in case discussions with the Supervisor and Program Director to evaluate the need for financial assistance.

Compile all necessary documentation to support in-house subsidy applications for clients.

Direct clients to community resources, public benefits, and entitlements.

Maintain precise records through case files and the designated digital database.

Oversee the arrangement, coordination, monitoring, and delivery of services to Homebase program participants.

Uphold the rights of program participants and perform additional duties as assigned by Supervisors or the Program Director.

REQUIRED QUALIFICATIONS
Bilingual proficiency in English and Spanish is essential.

Experience in non-profit sectors and a strong commitment to social equity are necessary.

Exceptional communication skills with the ability to advocate effectively.

Capability to work autonomously while also collaborating within a team.

A proactive mindset, with skills in problem-solving and relationship-building.

EDUCATIONAL BACKGROUND
An Associate's degree is required; a Bachelor's degree is preferred.
