Supportive Housing Case Coordinator

Work Schedule: Full-time - 35 hours per week; flexibility for evening and weekend hours may be necessary
Salary: $50,000 annually
The Sea Rise Supportive Housing Program encompasses a total of 199 units, with 72 designated for supportive housing. This includes 35 units for individuals with serious mental health challenges, 20 units for homeless young adults aged 18-25, and 17 units for those with intellectual and/or developmental disabilities. On-site support services are delivered by Brooklyn Community Services (BCS), offering comprehensive case management, vocational training, employment assistance, and additional support services.
Key Responsibilities:
- Facilitate access to client services, including referrals for specialized care (medical, mental health, substance use, etc.), appointment coordination, and other necessary processes to support residents in achieving their Individualized Treatment Plan goals and enhancing their self-efficacy.
- Timely completion of all required assessments and service plans.
- Document progress notes within 24 hours following individual sessions.
- Conduct weekly meetings with all clients on caseload, or as directed by the Program Director.
- Ensure accuracy and timeliness of all clinical and administrative documentation in accordance with program policies and regulatory standards.
- Provide crisis intervention services and complete incident reports as per regulatory requirements.
- Assist residents in their homes and the community to improve daily living skills, including but not limited to cleanliness, personal hygiene, meal preparation, budgeting, and navigating healthcare systems.
- Accompany clients to appointments as directed by the Program Director.
- Collaborate with property management to address client housing needs and maintenance issues.
- Conduct regular apartment inspections as required.
- Work with management to assist clients in the recertification process for their housing units.
- Provide case management coverage as needed to maintain quality care for residents.
- Facilitate weekly group sessions on topics such as mental health awareness and substance abuse prevention.
- Participate in fire drills and other safety protocols.
- Assist with the coordination of resident move-ins and move-outs.
- Engage in community recreational events to foster socialization skills among clients.
- Support community residents and local social programs.
- Attend all required meetings, trainings, and case conferences.
- Complete additional tasks as assigned by the Program Director.
Qualifications:
- Bachelor's degree in social work or a related human services field, with at least two years of experience working with homeless youth in supportive housing settings.
- Certification as a CASAC or CASAC-T is preferred.
- Strong written and verbal communication skills.
- Proficient in basic computer and data entry tasks.
- Background check required, as the position involves direct interaction with clients and the public.
