We offer over 180 integrated programs across various sectors, including Education, Family Support, Employment Services, Health, Housing, and Legal Assistance, reaching more than 73,000 individuals and families annually.
CAMBA's Housing Ventures (CHV) focuses on creating affordable housing solutions for low-income individuals and those with special needs in Brooklyn and New York City. Our initiative aims to develop 1,000 units of permanent housing over the next decade, providing a comprehensive range of housing and supportive services to lower-income families. In collaboration with Central Brooklyn Community Services Corp., we are launching a supportive housing project in the Bushwick area, designed to accommodate a diverse tenant population, including low-income workers, individuals with mental health challenges, the formerly homeless, and those living with HIV/AIDS.
Position: Case Manager
Reporting To: Program Manager
Key Responsibilities:
- Establish and maintain professional relationships with clients while ensuring confidentiality.
- Adhere to Universal Precautions and Standard Protocols.
- Comply with all relevant security and privacy policies to protect client information.
- Review documentation to confirm client eligibility and maintain accurate client files.
- Conduct initial assessments and ongoing evaluations of client needs.
- Collaborate with clients to develop and update independent living plans with achievable goals.
- Assist clients in accessing community resources and making appropriate referrals.
- Support clients in overcoming barriers to achieve their goals and promote self-advocacy.
- Monitor client progress through regular communication and document findings.
- Participate in case conferences to discuss client status and strategies.
- Follow up with clients and referral organizations regarding progress.
- Prepare required reports on client status and program outcomes.
- Recommend case closures when clients achieve their goals or become ineligible for services.
- Act as an advocate for clients with external organizations regarding education, healthcare, and legal matters.
- Assist clients with appointment scheduling and may provide transportation to appointments.
- Help clients complete applications for benefits and entitlements.
- Conduct home visits to evaluate living conditions when necessary.
- May assist in creating marketing materials for the program.
- Input client data into automated systems for tracking progress.
Qualifications:
- Bachelor's degree in a relevant field (e.g., B.A., B.S.W.).
Additional Requirements:
- Flexibility in scheduling may be required to cover shifts until evening hours.
Compensation: $45,000 annually. Salary is determined based on various factors including experience and education.
Status: Full-time (35 hours per week)
